Feedback & Complaints
Your experience as a service user, relative or carer is important to us. All feedback, whether it is a compliment, a complaint or a recommendation about any of the services we commission, will be welcomed and will help us to identify what we are doing right but also where we need to make changes to improve the experience of future patients.
Freedom of Information (FOI)
Any person can make a request for information under the Freedom of Information Act.
To submit a Freedom of Information request you can either:
Subject Access Request
If you wish to make a request to access the information the CCG holds that identifies you (Subject Access Request) please complete and return the SAR Form with any relevant ID Documents that are required for proof of identify.
If you require access to your medical records then please make a subject access request to the service that provided your medical care direct e.g GP Practice, Hospital department, District Nurses etc.
